A three-part field report

I. The care pack

Every shared-care request leaves us with a packet attached: drug names, target ranges, dosing schedule, monitoring intervals, and a direct email for the lead nurse. We learned that the absence of any one of these multiplies the chance of a no.

II. The next practice

When a practice declines: a curated list of trans-friendly practices within commute range. Nobody should have to fight this twice.
