Transfeminine hormone therapy, also known as feminising HRT, combines estrogen with testosterone-suppressing medications (anti-androgens or blockers) to produce physiological changes that align with a feminine gender identity. It is the most common medical intervention sought by trans women, transfeminine, and many non-binary individuals.

Estrogen drives the development of secondary sex characteristics associated with femininity: breast development, softer skin, redistribution of body fat to the hips and thighs, reduced body and facial hair growth, and emotional shifts many patients describe as profoundly positive.

Estrogen produces changes gradually over months and years. The timeline below reflects typical patterns. Individual results vary based on genetics, dosage, and delivery method.

### 1-3 Months

#### Skin Softening

Skin becomes softer and less oily as sebum production decreases. Many patients notice a more refined skin texture.

#### Decreased Libido

A reduction in libido is common early on, particularly when anti-androgens are introduced alongside estrogen.

#### Breast Buds (Thelarche)

Small, sometimes tender breast buds begin to develop beneath the nipple. This is a permanent change.

#### Mood & Emotional Shifts

Testosterone Suppression

## Anti-androgens & testosterone blockers

For most transfeminine patients, estrogen alone is not sufficient to achieve optimal feminisation. Anti-androgens suppress endogenous testosterone, allowing estrogen to work more effectively.

### GnRH Analogues

e.g. Leuprorelin, Buserelin, Triptorelin

GnRH analogues work by suppressing the pituitary gland's release of the hormones that stimulate testosterone production. They provide very effective testosterone suppression with a favourable safety profile.

-   Administered as monthly or 3-monthly injections or subcutaneous implants
-   Highly effective at suppressing testosterone to very low levels
-   Allow lower doses of estrogen to achieve optimal feminisation
-   Frequently used in younger patients and those beginning transition early

Your Pathway

## From enquiry to your first prescription.

Most patients reach their first prescription about three months after their first video consultation, provided their bloods and GP medical summary arrive without delay.

Diagnosis

### A signed diagnosis letter

UK law requires a written diagnosis of gender dysphoria or gender incongruence from an HCPC-registered psychologist or GMC-registered psychiatrist. We can recommend specialists if you don't have one yet.

Video consultation

### A 45-minute remote call

Once your diagnosis is verified, you book a video consultation (typical wait 4-8 weeks). We discuss your medical history, goals, fertility options, and the path ahead. No bloods required at this stage.

Bloods & paperwork

### 4-6 weeks of prep

Between calls you arrange the baseline blood panel, request a medical summary from your GP, and complete the consent forms. The faster you submit, the faster the next step.

In-clinic appointment

### First prescription, same day

Your first appointment is always in person, in London, Stafford, or Exeter. We check blood pressure, do an in-body analysis, review bloods and history, sign consent, and if everything is safe, issue your first prescription that day.

After your first prescription

### Care continues, flexibly.

Shared care attempt

Within 2-3 weeks we send a shared-care agreement to your GP. If they sign, you get hormones on the NHS for ~£9.90 per item. If they decline, your prescriptions stay private and are included in your monthly plan.

Follow-up reviews

Reviews every 6 months for the first two years, then annually once stable. Most can be remote by video; only the very first appointment is mandatory in person.

Pause whenever you need

Therapy can be paused, restarted, or transferred to NHS care at any time without penalty. We'll always send a final summary letter to your GP.
