Diagnosis / Referral Guide

To begin care with our team of experienced clinicians, you must first have a diagnosis/ referral stating a clear diagnosis of gender dysphoria. For this to be accepted, we have a set of criteria listed below to assist patients in making their first booking.

What information is required in your diagnosis?

For your diagnosis here is a breakdown of what is required of your diagnosis / referral form:

  • A None Editable Document Format
  • Date of Assessment
  • Patient Details
  • Introduction of Clinician Detailing Prior Experience Within The Field
  • Appointment History
  • Patient History
  • Summary Of Current State Of Patient
  • Past Medical History Of The Patient
  • Clear Detail Of Diagnosis
  • Proposed Care Plan (Agreed With Patient)
  • Clinicians’ Signature
  • Name Of Clinician

Optional But Regularly Included Information

  • Address Details Of Recipient 
  • Family History
  • Collateral History

It is important your diagnosis or referral is carried out by an Health Care & Professions Council (HCPC) registered or General Medical Council (GMC) registered Psychologist or Psychiatrist. This is so we can validate the credentials of the clinician. You can check that your clinician is registered by either seeing if it has been stated on your form, or checking the public registers listed below.

While we do accept diagnosis and referral forms from clinicians practising outside of the UK, these would need to be evaluated by our clinical team and the patient would need to have been residing in the country the clinician is practising within during the time of diagnosis.
