Book Your Follow Up Appointment
The following details are required prior to booking your follow up appointment:
Blood tests results are required to be uploaded via the link provided prior to your follow up appointment. It is vitally important that we receive these results otherwise we will unfortunately be unable to make any recommendations or adjustments to your prescription.
Please check our last correspondence to you and your GP for the blood tests that we have requested for your follow up appointment.

Please be aware, that blood results must contain the date the results were collected, the patient’s name and date of birth.
(Please note you must be an existing patient to make a request)
Please note, patients must be seen in the clinic at least once per year to comply with safe prescribing guidelines. If your last appointment was a video appointment, you are required to be seen in the clinic. If your appointment type doesn’t match, your appointment request will be rejected.
