About GHC

The Gender Hormone Clinic is a nurse-led service providing gender-affirming hormone care to adults. We opened in February 2023 with around 1,200 patients and now care for close to 4,000, with 80 to 120 new patients asking to join every month.

We're growing because patients need us. You'll join a team of about 19 across Stafford, London and Exeter, led by experienced nurse prescribers and supported by a specialist multidisciplinary team. We put compassionate, evidence-informed care, patient safety and good clinical governance first.

The role

The patient pathway:

  1. Step 1

    Video history

You assess eligibility, then review the patient's GP summary and baseline bloods.

  2. Step 2

    Consent appointment

A one-hour, face-to-face appointment in clinic covering benefits, risks and alternatives.

  3. Step 3

    Prescribing

You start treatment after the in-person appointment, within our clinical protocols.

  4. Step 4

    Follow-up

Six-monthly reviews for the first two years, then yearly once stable, including blood results and dose adjustments.

A typical day is about four patient appointments, alternating with admin time. You'll spend 2 days a week at our London clinic and 3 working from home. This isn't a fully remote role.

Key responsibilities

Once signed off on our protocols, you will:

  • Run video history and eligibility assessments, taking medical, medication and family histories.
  • Lead face-to-face consent appointments, including physical observations and a clear explanation of benefits, risks and alternatives.
  • Prescribe, start and adjust gender-affirming hormone therapy within our clinical protocols.
  • Review and interpret blood results, escalating anything that needs further investigation.
  • Monitor treatment response, side effects and complications at follow-up.
  • Give clear patient education, health promotion and safety-netting.
  • Write accurate clinical records and letters to GPs, including shared care.
  • Recognise clinical, psychological and safeguarding concerns and escalate them.
  • Take part in clinical governance, audit, training and CPD.

With experience, you can manage a growing share of the patient pathway independently.

Pay

The salary we advertise is the salary we offer.

  • Salary: £60,000 to £65,000

For nurses with a V300 (or equivalent) prescribing qualification.

  • Where you start in the range

Depends on your experience, and we'll explain your figure in writing.

On a 37.5-hour week, £60,000 works out at about £30.77 an hour and £65,000 at about £33.33 an hour.

Progression

We're growing fast, so there's real room to grow with us. Your responsibility increases as your experience in gender-affirming care builds.

  1. 1

    First year: settle in

Train on our protocols, then prescribe for a set caseload with senior nurse prescribers on hand.

  2. 2

    With experience: more independence

Manage a growing share of the patient pathway independently, including more complex cases.

  3. 3

    Over time: a senior role

Step into a more senior nurse prescriber role, supporting and mentoring newer nurses and prescribers as the clinic grows.

Training and support

You don't need experience in gender care. We'll train you in our protocols before you prescribe independently.

  1. First weeks

Shadowing senior nurse prescribers, then running your own history appointments.

  2. First year

A set caseload while you settle in, with no pressure to take on more.

  3. Throughout

Senior nurse prescribers on hand during clinic hours, weekly clinical training, and a multidisciplinary team (endocrinology, psychiatry and psychology) for complex cases.

Your induction and training cover

  • Gender-affirming hormone therapy and the medicines used
  • Clinical assessment and ongoing monitoring
  • Interpreting relevant blood results
  • Recognising and managing complications
  • Clinical protocols and escalation pathways
  • Shared care and communication with GPs
