Reading WPATH SOC 8.

A patient-friendly walk-through of the international clinical guideline that shapes our care: what it says, what it changes, and what it means for you as a patient.

What it is

The international standard of care for trans health.

The Standards of Care, version 8 (SOC 8) is a clinical guideline published by the World Professional Association for Transgender Health (WPATH) in 2022. It runs to about 250 pages, was written by an international panel of clinicians and researchers, and is the document most gender-affirming services around the world point to as the basis for their practice.

It is a guideline, not a law. It does not replace local regulation: in the UK, that's the General Medical Council, the Nursing and Midwifery Council, and the Care Quality Commission. But it sets out the evidence-based framework most reputable clinics, including ours, work within.

SOC 8 is freely available on the WPATH website. The summary below is our reading of it for adult patients in the UK.

Theme 1

Informed consent

Adults capable of consent should be supported to make informed decisions about their own bodies. The clinician's job is to give clear information and assess capacity, not to gatekeep based on identity.

Theme 2

Individualised care

There is no single transition pathway. Hormone therapy, surgery, voice training, and social transition are all options people may pursue in different combinations and at different paces.

Theme 3

Non-binary inclusion

For the first time, non-binary identities are explicitly recognised, with dedicated guidance on goal-directed and microdose approaches alongside binary pathways.

Theme 4

Safe monitoring

Regular blood work, cardiovascular monitoring, and bone density review are baseline. SOC 8 sets out which markers to track, how often, and what targets to aim for.

In your assessment

Your initial appointment is a clinical conversation, not a tribunal. We assess capacity to consent, talk through goals, and confirm there's no medical reason to wait. There is no requirement to "prove" your identity.

In your prescription

We choose medications and doses based on your stated goals and your bloodwork, not a fixed protocol. If you want a slower, partial, or fully binary path, we plan around that.

In your follow-up

Bloods, cardiovascular markers, mental-health check-ins, and bone-health review at the intervals SOC 8 recommends. Anything outside the expected range gets discussed and addressed.
