Fertility preservation.

Hormone therapy can permanently affect fertility. We strongly recommend arranging egg or sperm storage before starting HRT. We don't organise preservation directly, but we can support your application.

Why it matters

Hormones and fertility.

Both testosterone and oestrogen therapy can significantly impact fertility, and these effects are not always fully reversible. If there's any chance you may want biological children in the future, we strongly recommend preservation before starting HRT.

Preservation isn't mandatory, but it must be completed or formally declined before your in-clinic appointment so we can issue your first prescription. Delays do not block your video consultation; they postpone the in-clinic stage.

HRT is not a contraceptive. Pregnancy remains possible on testosterone therapy until ovulation is fully suppressed (and sometimes still afterwards). If contraception is a concern, raise it at your initial consultation.

Routes to preservation

NHS or private.

NHS via your GP

Up to 10 years free storage

NHS-funded fertility preservation is offered through your local Integrated Care Board (ICB). Your GP makes the application; we can write a supporting letter explaining the impact of HRT on fertility.

Find your ICB at nhs.uk/find-your-local-icb. Find HFEA-licensed clinics at hfea.gov.uk.

Wait times: typically 6+ months. If your application is denied, ICBs have appeals processes, and we can supply template arguments.

Private clinics

Faster, ~£5-10k + annual fee

Private fertility clinics can move much faster than the NHS route, often weeks rather than months. Costs typically £5,000-£10,000 plus annual storage fees.

We recommend Harley Street Fertility Clinic (hsfc.org.uk) among others. Confirm pricing and timeline directly.

How we support

Letters, scheduling, and advocacy.

  • Supporting letters for your GP to take to the ICB, outlining the fertility impact of HRT and the urgency.
  • Scheduling flexibility, so your in-clinic appointment can be delayed without penalty until preservation is complete or formally declined.
  • If your ICB declines funding, we can supply template appeal arguments and complaint letters citing equality and human rights legislation.
  • During hormone consultations, we explain contraception, sperm-recovery timing if you pause therapy for banking, and what to expect post-storage.

Note: stopping blockers for sperm or egg banking may require time for hormone recovery; success varies by individual and timing.
