Consent to disclose

Authorise a named partner, carer, or family member to speak with us about your care. Both identities are verified before any disclosure.

What this form does

Authorise someone to act on your behalf.

By default, our admin and clinical staff only discuss your care with you. If you want a partner, parent, carer, or other named person to be able to call us, schedule appointments, or discuss your records, we need a signed consent-to-disclose form on file.

Both identities are verified at the start of any conversation. Without consent on file, we explain only general policy, not your specific care.

Consent can be added, varied, or withdrawn at any time by emailing or by submitting a fresh form.
