Why it matters
Avoid prescribing gaps when you move.
Switching GP never pauses your treatment with us. Submit your new details and we forward your historic correspondence and shared-care forms to the new practice, confirming once sent.
We send the shared-care pack to your new practice within 2-3 weeks, but GP approval itself can take 8-12 weeks. Frequent moves can create prescribing gaps, so many students keep their home GP for continuity through term.
NHS-number changes are usually handled by your GP. Tell us when it happens; shared care normally remains valid unless the new practice insists on fresh paperwork.
